What is a sleeve gastrectomy?

Sleeve gastrectomy is a keyhole surgical procedure for weight loss that involves stapling off and completely removing a portion of the stomach, leaving a small tube with the capacity of approximately 200ml. Laparoscopic sleeve gastrectomy surgery, performed by our Perth based surgeons, takes 1-2 hours. Patients usually stay in hospital for 3 nights and are able to return to work in 1-2 weeks after their gastric sleeve surgery.

What are the advantages of sleeve gastrectomy?

Sleeve gastrectomy is now a fairly routine procedure. We have over 15 years of data and follow up of sleeve patients.

I have personally performed 3000 sleeve gastrectomy operations since 2005 . Complications are fairly rare. Weight loss is good . It depends a bit on your starting BMI . If you have a BMI of 35 you could lose all your excess weight. If your BMI is 45 you are probably going to lose 70-80 % of your excess weight. Most weight related issues would be expected to get better.

Many diabetics are off their meds quickly, or at least less meds. Sleep apnoea would be expected to get rapidly better. 60 % of patients with hypertension are off their blood pressure meds at 6 months. Arthritis and joint issues improve with rapid weight loss. Most patients would have pretty good quality eating 6-8 weeks later . If you are thinking about surgery and planning time off you would probably need 2 weeks off work and could return to all exercise about 3 weeks later.

Am I eligible for gastric sleeve surgery?

Gastric sleeve , or sleeve gastrectomy is by far the most popular bariatric procedure both in Australia and around the world. It suits most patients with a BMI under 50 without significant reflux. It gives predictable solid weight loss and generally good quality eating. Nutritional problems post op are fairly rare. It is not particularly complicated to perform, although it needs to be done carefully. Most patients recover quickly and are back at work within 2 weeks.

If you have a BMI over 50 (which you can work out on our website) you might not lose quite enough weight with a sleeve and may need to look at a gastric bypass procedure. Reflux can get worse after sleeve gastrectomy as the stomach is made smaller and the pressure inside can be higher and cause more reflux issues. Other than that most patients with a BMI of 35 + will likely be a good candidate for a sleeve.

After the Gastric Sleeve Procedure

After the gastric sleeve patients would hope to achieve a 65% reduction in their excess weight in a year due to a significant reduction in appetite and portion size. Many co-morbidities such as Type 2 diabetes, hypertension, sleep apnoea and joint problems would be expected to get better or disappear.

It is important to limit snacking to reduce stretching, perform weight bearing exercises at least twice a week, and take vitamin supplements iron, calcium and B12 after a laparoscopic sleeve gastrectomy. A dietician can advise on the proper proportions, nutrition and meal planning. Follow-up with the surgeon is typically six-months after the surgery and then every year for at least 5 years.

Gastric Sleeve vs Gastric Bypass

If your BMI is over 50 gastric bypass may give you better weight loss than a sleeve. If you have significant diabetes, and particularly if you are now on insulin injections you may get better diabetic control opting for a bypass rather than a sleeve. If you have bad reflux issues to start with and are taking regular meds for this already we may discuss bypass rather than sleeve. If you live abroad or in a remote area sleeve gastrectomy may be preferable than a bypass as less nutritional support and follow up blood tests are required. We can talk you through all these important decisions and tailor the approach to your individual circumstances.

Sleeve gastrectomy guidelines

These are some guidelines to help achieve long term weight management.

  • Eat regularly. Avoid skipping meals and plan to eat 3 small meals plus 3 snacks per day.
  • Ensure all food consumed is nutritious.
  • Eat slowly and chew food thoroughly.
  • Sense fullness and stop eating.
  • Avoid carbonated drinks for up to 6 weeks post surgery.
  • Include 2 sessions of weight bearing exercise per week.
